This 3-month Hormone Initiation Package provides structured medical care for women who prefer FDA-approved hormone therapies filled at retail pharmacies, or who want short-term guidance before transitioning to ongoing membership care. All treatment is provided only after medical evaluation and determination of clinical appropriateness. This package represents a structured medical care program — not medication access. Prescribing is based on medical necessity and clinical evaluation.
This practice operates on a cash-pay model and does not submit insurance claims, prior authorizations, appeals, or insurance-coded documentation. Patients are responsible for coordinating insurance coverage directly with their pharmacy. Insurance coverage for FDA-approved medications is not guaranteed and may be denied by your plan.

Hormone therapy requires time to initiate, stabilize, and adjust safely. This 3-month package reflects the minimum period needed for responsible medical evaluation, treatment initiation, and assessment of response. This is a 3-month medical care commitment and is not intended as a one-month trial. Once a month of care has begun, that month is non-refundable, as clinical services and medical decision-making have already been provided.
At the end of the 3-month period, patients may transition into a Women’s Hormone Membership if ongoing care is desired, which includes care utilizing compounded hormone therapies. Alternatively, patients may choose to repurchase the 3-month Hormone Initiation Package if they prefer to continue care using FDA-approved medications filled at retail pharmacies. If no further care is pursued, treatment concludes at that time.

Multi-Month Commitment Savings
Patients who prefer longer-term care may choose discounted commitment options for select monthly memberships only, with payment made in full at the time of enrollment.
These savings apply to ongoing monthly hormone replacement therapy (HRT) memberships and monthly medical weight loss memberships.
6-month commitment: 5% off the total membership cost when paid in full
12-month commitment: 10% off the total membership cost when paid in full
Multi-month savings do not apply to:
3-month programs or packages

Monthly memberships are billed on a recurring basis using the payment method on file.
Memberships may be canceled at any time with at least 7 days’ notice prior to the next scheduled billing date.
Cancellations take effect at the end of the current billing cycle.
Membership fees are non-refundable once charged, as clinical services, provider availability, and prescription management begin immediately.
If a payment method fails, is declined, or is removed, the membership will be considered canceled and no further prescriptions or refills will be issued.
Ongoing medical care and prescription management require an active, paid membership.

We do not bill Medicare, Medicaid, or private insurance plans and do not submit insurance claims, prior authorizations, appeals, or insurance-coded documentation.
Some laboratory testing may be billed through insurance when available. If insurance coverage is not available, discounted self-pay lab options are typically offered.
Because this practice does not provide insurance-coded documentation, insurance coverage for prescribed medications is not guaranteed. Patients who choose to use insurance for FDA-approved medications filled at retail pharmacies assume the risk of coverage denial.
When clinically appropriate, prescriptions may also be coordinated through trusted U.S.-based compounding pharmacies to allow for personalized dosing and convenient home delivery.
